<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>An employee suffered a cut to the left middle finger while cutting insulation because he was wearing gloves that were inappropriate for the task.</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On February 2, 2016 at approximately 09:45, a Carter Insulation employee at the Waste Solidification Facility (WSF) at the Savannah River Site (SRS) was using a knife to cut mineral wool insulation for penetration packing.
The knife was in the right hand and the mineral wool was in the left. The employee pushed the knife through the mineral wool and received a cut on his left middle finger.
The employee was wearing gloves at the time, however, they were not specified for the task. (See Attachment for a picture of the glove used for this event and the correct glove that should have been worn.)</pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>The employee pulled his glove off, wiped the blood away with a paper towel, placed a Band-Aid on his finger, re-donned the gloves, and continued to finish the task. Once complete, the employee cleaned the area and notified Baker Supervision of the incident.</pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
The employee notified his supervision and all activities related to the work were halted.
Baker Construction removed all gloves other than those specified as puncture resistant, from the job site.</pre>
<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>Jefferson Lab's PPE Subject Matter Expert has read and provided the following comment:
Cutting insulation occurs daily here at Jefferson Lab. Some examples of insulation used at Jefferson Lab include fiberglass building insulation, foam piping insulation, and REFRASIL for cryogenic systems.
At a minimum safety glasses and cut resistant gloves should be worn. Other PPE may be required depending on the material being cut. Cut resistant gloves and safety glasses are available in the Jefferson Lab Stockroom.
Sharps must be kept closed when not in use and placed in a sharps container for waste disposal when not longer usable. Sharps containers are available in Building 35.</pre>
