<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>Prior to using any machine tool, ensure you are familiar with its operation and limitations. Do NOT use any fixed/stationary power tool with unguarded rotating components. If observed, immediately notify your supervisor and tag the equipment out of service.</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>Recently, a machine guarding safety surveillance performed in the Nuclear High Hazard Operations Project, identified four drill presses needing review. Three of the four lacked guarding around the chuck/drill bit. One of the four was fitted with a removable plexi-shield which is considered acceptable for continued operations.</pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>Following this finding, an extent of condition review of all drill presses across URS CH2M Oak Ridge LLC (UCOR) revealed that only two of a total 14 had functioning guards.
Occupational Safety and Health Administration¿s (OSHA¿s) Machine Guard standard, general requirements for all machines, Title 29 Code of Federal Regulations (CFR) Part 1910.212(a)(1) states: "One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, and flying chips and sparks."
To clarify, this general standard does apply to drill presses. An OSHA letters of interpretation (see attachment) confirms the requirement. </pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
A) Tag "out-of-service" all non-compliant drill presses until it is determined if it is needed,
or
Provide an explanation/justification of another compensatory measure that supports ongoing use. (e.g. isolating the area around the equipment while remotely controlling operation.)
B) Immediately guard all drill presses currently in use to meet 29 CFR 1910.212(a)(1).
C) Provide a toolbox meeting for all UCOR employees to serve as a reminder for those who use fixed/stationary power tools, that guarding provides protection to the operator.</pre>
<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>Jefferson Lab recognizes that machine tools are inherently risky and has determined that use of a machine tool carries an unmitigated Risk Code 3. In accordance with "ES&H Manual 6121 Machine Tools" an Operational Safety Procedure (OSP) is written for each machine tool and read by machine operators prior to use.</pre>
