<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>Wearing improper laser eye-wear is like wearing no laser protective eyewear (LPE) at all. When using LPE, the user must (1) ensure it is not broken; (2) ensure a proper fit; and (3) ensure it provides adequate optical density for the wavelength. LPE must always be inspected before use in fully light and never be stored or placed near other types of eye-wear (e.g., sunglasses, reading glasses, etc.). LPE must only be in one of three locations: (1) in a designated storage location containing only the correct LPE for the laser being worked with; (2) on a lanyard worn by the person using the LPE; or (3) properly donned over the user's eyes.</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On June 14th, while working with other researchers on an experimental setup, a visiting graduate student received exposure from a laser. At approximately 11:45am, the work group was told to don LPE. The graduate student reached down, picked up, and donned what he thought was his LPE from a work table. After the confirmation that everyone had donned their LPE, the room was darkened and the laser¿s shutter opened to admit the beam. The student walked over to the laser's shutter area to toggle a switch when he saw a green spot of light on an optic. This lasted for approximately two seconds. He briefly pondered why he was able to see the green light, but concluded it was probably okay because he had seen diffuse light before in his university laboratory when working with continuous wave lasers. He then went over to a computer to do additional work.
About 15 minutes later he realized he was seeing gray light rather than orange light coming off the computer screen. Knowing that a computer screen should appear orange when viewed through the appropriate LPE he left the room to investigate and discovered he had been wearing another researcher's personal sunglasses.

<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
The graduate student notified the other workers in the area and the JLF Manager/Authorizing Individual (AI).
The graduate student was sent to Health Services to be seen by an ophthalmologist. No eye damage was detected.
A stop work initiated and remained in effect until a re-start meeting was held. All workers were informed of immediate corrective actions to be taken, including increasing unauthorized personal items and other reflective surfaces enforcement; strengthening work control documentation to specifically require that sunglasses be secured at all times; and provide frequent reminders to visiting researchers about strict enforced of LPE requirements.

Jefferson Lab shared the original DOE Occurrence Report with all Laser Owners and workers.
Jefferson Lab has determined that Class 3B and Class 4 laser light poses an unmitigated Risk Code>2 and requires a separate work control document be reviewed and approved for each laser and the area where it is located. ES&H Manual Chapter 6410 Laser Safety Program defines the hazard issues associated with lasers and the minimum mitigation required before activities involving lasers can commence.
