<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>Portable eye wash station, used for the flushing de-foaming liquid from a worker¿s eye, was identified as the cause for debris found in the eye during treatment at the hospital.</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On the afternoon of September 1, 2015, a Navarro worker at Building 06-909 was conducting evaluations of a new lead screening kit using mock-up drilling fluids. The worker was wearing safety glasses with side shields as recommended. Activities included opening a 5-gallon bucket of de-foaming liquid and lifting the bucket to nest inside another bucket. When the open bucket settled into the lip of the lower bucket, liquid splashed out and got under the worker¿s safety glasses and into the left eye.
The worker immediately went to the nearby eye wash station and started flushing the eyes. The worker was then taken to a nearby fire/aid station for evaluation and transported to a hospital in Las Vegas, Nevada. During the eye flushing at the hospital two small insects, approximately the size of pepper flakes, were flushed from the eye.
The worker was released after treatment on the same day for full duty with no identified injury.

<h2 style="white-space:nowrap">Analysis</h2>
<pre>Following the incident critique, Navarro personnel confirmed two eye wash stations located in B 6-909 contained foreign materials (e.g., bugs, organic matter). The eye wash station inside the UGTA lab trailer did not contain foreign materials.</pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Below are the consequences of the above event. They are provided for information and discussion purposes only.
Personnel emptied, rinsed and refilled the two eye wash stations in NNSS Building 6-909. As a recommendation from the HS Manager, the top fill cover was taped to provide a seal to minimize the potential for foreign materials to enter the eye wash water.</pre>
<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>Jefferson Lab¿s Safety Wardens are responsible for weekly eye wash inspections. Qualifying inspections include ensuring the apparatus is clean and water flow is adequate. The Safety Warden Program is outlined within ES&H Manual Chapter 2500 Safety Warden Program. Safety Wardens contribute to the efficient implementation of effective safety regulations and best work practices. Each is assigned a specific area and provides ready assistance to workers to help facilitate resolution of safety issues.</pre>
