<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>Visually verify the exact location of a new conduit immediately
following the initial installation, especially if the conduit is
installed through a blind penetration. In addition, re-verified the
location prior to final connection and tie-in with the electrical
source, especially if there was a time period between the initial
installation and the final tie-in.
</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On May 27, 2016, a subcontracted electrician, using a horizontal band
saw, mistakenly cut into a 3/4-inch diameter conduit. The conduit
contained an energized 120 volt alternating current (AC), 20 ampere
conductor powering a fire alarm panel. The electrician intended to work
on a different conduit with similar size and appearance located
approximately 11-inches away and 3/4 inch higher, which had been
installed a few days earlier.
The electrician was wearing the required Personal Protective Equipment
(PPE) and was standing on a non-conductive fiberglass ladder.Upon
contacting the energized conductor, the circuit breaker opened and the
fire alarm panel switching to battery back-up power. The electrician
heard a noise from within the conduit and ceased operations. He was not
injured or shocked.</pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>Causal analysis identified the following factors:
1.Work Check was less than adequate (LTA) / Emphasis on schedule
exceeded emphasis on methods: Subcontractor employee did not visually
verify location of newly installed conduit.
2.Time Delay caused LTA actions / Job performance and self-checking
standards not properly communicated: Several days passed between the
initial installation of the conduit and the final tie-in with the
electrical source resulting in a loss of process knowledge.
3.Task Progress / status was not adequately tracked: Work status was not
adequately tracked / communicated between the general contractor and
subcontractor.</pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>The following are provided for discussion purposes. It demonstrates what was done to prevent future occurrences:
1.The Job Safety Analysis (JSA) wase updated to reflect a requirement
that the location of new conduit be visually verified by the
subcontractor employee performing the work immediately following initial
installation.
2.The JSA was updated to reflect a requirement that the location of the
new conduit be re-verified by the subcontractor employee performing the
work if any period of time elapsed between the initial installation and
the final connection and tie-in with the electrical source.
3.Meeting minutes and a signed attendance sheet showing that both the
general contractor and subcontractor employees met to discuss the
incident and review the additions made to the JSA in an effort to better
track future work involving new conduit installation.</pre>
<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>Jefferson Lab's Electrical Safety Engineer was contacted to discuss preventive measures. Abiding by Jefferson Lab's Lock, Tag and Try program, work planning and communication between workers is the best way to prevent occurrences.</pre>
