                    <h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
                    <pre>Walking surface conditions that have existed for years does not automatically mean they are adequate. Maintain a questioning attitudewhen inspecting facilities or work areas for potential hazards. </pre>
                  
                    <h2 style="white-space:nowrap">Discussion of Activities</h2>
                    <pre>An employee incurred a significant tendon injury and hairline fracture of the wrist when his foot slipped on some rust and slid off a 19-inch-wide aluminum scaffold walk-board onto the floor six inches below.  The employee was part of a group walking down a facility's basement prior to the repair of the safety eye-wash and shower lights. 

The employee was transported to the on-site health services and subsequently an orthopedic surgeon.

                    <h2 style="white-space:nowrap">Analysis</h2>
                    <pre>The subject walk-board system had been installed over 15-years ago to provide a walking surface that remained above a basement floor where ground water infiltration and spills/leaks from wastewater process tanks accumulated.  When the decision was made to add the walk board system, local maintenance personnel selected the 19-inch wide aluminum scaffold walk boards because they provided a better walking surface than wood planks and were resistant to degradation.  There was no evidence of an engineering or safety review.</pre>
                  
                    <h2 style="white-space:nowrap">Recommended Actions</h2>
                    <pre>1.       Existing facilities were reviewed for compliance with expectations for providing safe effective walking surfaces.
2.      Ensure new walking surfaces, are reviewed by designated Engineering or Safety personnel who are trained in establishing acceptable materials and specifications.
3.      When performing task walk-down include walking surfaces, stairs and other permanently installed access equipment and maintain a questioning attitude.

        <h2 style="white-space:nowrap">JLab Preventive Measures</h2>
        <pre>In accordance with ES&H Manual Chapter 3110 Facility Design and Modification Review - ES&H reviews are required for designs for new and modification to existing facilities including:
-       New structures;
-       Architectural, structural, mechanical, plumbing or electrical system modifications to existing facilities; and
-       Changes in building use.

Considerations include:
-       Life, safety, and fire protection.
-       Environmentally responsible construction techniques.
-       Restoration and protection measures to enhance long-term stewardship of natural and cultural resources throughout facility's operation, closure, and post-closure life cycles.
