<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>In accordance with DOE Standard 1090-2007: Hoisting and Rigging Standard ¿ slings (rigging equipment used to attach a load to a lifting/hoisting device) are to be inspected prior to use. During a pre-use inspection three round slings were found to have knots concealed by a covering, designed to protect the load-bearing core (reference ASME B30.9 2014 section 9-6.9.5 (g)). The coverings were in good shape with no wear or chaffing. To be safe the slings were tagged [out of service] until further analysis could be conducted. </pre>

<pre>LLNL procured new slings from the same manufacturer (Carpenter Rigging) and another manufacturer (Lift-It) to compare with the existing tagged slings. A side-by-side comparison verified that all the slings had knots in the exact same location (See Attached Figure 1).
To confirm the safety of the slings, the covers were carefully cut (without damaging any of the core yarns) to expose their internal constructions for comparison (Figure 2). It was determined that the knots were core-yarn-knots and an acceptable feature of sling construction. (Per ASME B30.9 2014 section 9-6.9.5 (g) rejection criteria ¿knots in the round sling, except for core yarns¿.) Therefore, slings should not be rejected due to knots located next to a yarn splice, as this is part of an acceptable manufacturing processes.

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Performed an extent of condition inspection, comparing all round slings. It was determined that all of the available round slings had the same features (knots) in the same location.
The manufacturer (Carpenter Rigging) was contacted for further assistance. The manufacturer¿s general manager confirmed that ¿the condition described is actually part of the splice made during the manufacturing process and is acceptable as noted in ASME-B30.9-2014, para 9-6¿.
Material Handling Personnel have been trained to coordinate resolution of questionable conditions and to use resolutions to improve process and performance standards in the future.

<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>In accordance with ES&H Manual Chapter 6141 Material Handling Equipment Program - Rigging, Cranes, and Hoists pre-use and periodic inspection of rigging and slings is required. Any anomalies are to be analyzed prior to used. </pre>
