Statement of Lessons Learned

Scientists relied on their experience, training, and safety protocols to immediately respond to an emergency. Together they provided a textbook example of safety culture when they helped an electrician avoid serious injury.

Discussion of Activities

Two electricians were performing troubleshooting activities on a laboratory hood when a container of sodium hydroxide, located in a cabinet underneath the hood, spilled, splashing one in the face. Two scientists, working in the lab took immediate action. One escorted the electrician to the eye wash station and to begin running water into his eyes, then called PNNLs - Single Point of Contact. The second retrieved a spill kit and called the Cognizant Space Manager (CSM). The second electrician took over at the eye wash station keeping the other calm.

Analysis

To prepare the area for the electricians to perform troubleshooting on a perchloric hood, a pipefitter loosened screws on the cabinet beneath the hood so it could side out easily.
When one of the electricians pulled on the hood he noticed that a shelf, holding a chemical container, was unstable. He asked the scientist escorting him if the chemical, sodium hydroxide, was hazardous, just as it tipped over, splashing him in the face. The electrician was wearing electrical PPE and safely glasses but was shocked by what happened and froze.
Another scientist, working nearby, saw the splash. She quickly guided the electrician, who kept his eyes closed, to the eyewash station and pushed his head under running water, then called the on-site medical staff. The escort scientist, set the chemical container upright, retrieved a spill kit from the hallway, and contacted the CSM. The second electrician remained with the other at the eyewash station, keeping him calm and reassured until assistance arrived.
The electrician was escorted to medical services and subsequently released to work with no restrictions, thanks, in part to the quick response by the scientists who were in the lab with him.

Recommended Actions

(These are provided for discussion purposes.)
Know how to respond in an emergency: The scientists responded to the chemical spill automatically, per their training. Be aware of what hazards exist in your work area and how to quickly and appropriately respond.
Wear appropriate PPE: In this lab eyewear was required and did its job to help protect the electrician. Always consider the hazards you work around and what PPE, required or not, may help to increase your safety, especially where you could receive unintentional exposure.
Respect lab rules: Although the requirement to wear PPE in some labs may seem unnecessary, abide by them to avoid injury.
Question hazards before performing work: The electrician asked about the hazards of the chemicals they were preparing to work around. This demonstrates two of the Safe Conduct of Research principles: "A questioning attitude is cultivated" and "Hazards are identified for every task every time".

JLab Preventive Measures

For Jefferson Lab chemistry areas, chemical operations are suspended and chemical containers closed and placed into chemical storage cabinets during maintenance periods. An informal orientation of the chemistry room¿s hazards is provided to maintenance workers and a full-time trained escort is required. Maintenance personnel must don appropriate PPE for the tasks they perform within the chemistry room.
