<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>1. Relocating of any storage cabinets, containers, or bins, previously stored in a radioactive material area (RMA) should be coordinated through the Radiation Control Department (RCD) to ensure legacy radioactive material (RAM) is not inadvertently transported and stored in an unposted/uncontrolled area.
2. Principal experimenters and Hall Coordinators should have a detailed post-run decommissioning plan that addresses the removal, transportation, disassembly and storage of radioactive materials associated with all experiments.

<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>The RCD Field Operations manager received a call from a Hall A Engineer, requesting assistance after discovering some radioactive items, (2) tagged; (1) untagged) in a storage cabinet located in an unposted area. Upon arrival, the items were collected, a survey of the cabinet was performed, and the condition was immediately corrected. </pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>The radioactive materials identified in this deviation report are legacy items. RAM# 1132 was tagged on 08/14/01; RAM# 1209 was tagged on 08/13/02, and the item controlled with tracking form #02-15 was initiated in 2002. All of the items were associated with an experiment in Hall A, and all items were originally stored in building 52 (Blockhouse). The RAM inventory in 2005 could not locate any of these items and was subsequently written-off, under the guidance of the RadCon Manager, and removed from the RAM database. The missing items were discovered in a storage cabinet in the Physics storage building which is within a Controlled area, but not posted for radioactive materials. RAM #1132 and #1209 when surveyed were indistinguishable from background. At the time, we could not locate any item(s) associated with tracking form #02-014. An immediate survey of the locker and trashcan was performed. No other activated items were discovered in the locker, however, there was a long foil in the trash can that was activated. After discussion, and further searching, it appears that this was the foil associated with the remaining tracking form. There was no RAM tag attached to the foil. Further discussions with the Hall A Coordinator, the Principal investigator (PI) for the experiment, and the Hall engineer, all suggest the primary cause to be loss of control of RAM
during the decommissioning of the Hall A experiment. After several years of inventory, the items, again were permanently categorized as missing. These foils were needed to perform post-run measurements, and the PI is excited to find that they are available. All items were transported to the RadCon waste area in the ESB (Bldg 23)

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Review operational procedures associated with posting and surveys with RCTs, and encourage RAM posting practices described therein when they are performing routine inspections, surveys in RMAs with very few tagged items, and when performing RMA inventories. Additionally, meet with Hall Coordinators to discuss this infraction and post-run RAM controls.</pre>
