<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>A worker was using an older State Disc Sander, procured in the 1990s, to sharpen a scraper that was being used on a concrete floor, when his right thumb came in contact with the sanding wheel resulting in a laceration and ultimately surgery.
When using equipment having rotational motion, hydraulic, or leverage force, abide by the instruction in the operator manuals and periodically revisit equipment warnings and limitations.

<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>In February 2017, a worker was using the State Disc Sander to sharpen a scraper. During the activity the scrapper got pulled in, pulling the worker's right thumb into the mechanism. The worker was wearing gloves, which were prohibited by the operator's manual, and safety glasses when the injury occurred. The worker lacerated their right thumb when it met the sanding wheel. </pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>The operator's manual was missing. Review of the hazards of machine operation were not available.
The warnings in the operator's manual included that for safe operation the object being worked on needed to be firmly on the table and be no longer than the table. These requirements were not met due to the area requiring sharpening, the size of the blade, and the angle of the edge.
The worker used the incorrect machine for the task and used it in an improper and unsafe manner.

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Regardless of the age of a machine, the operator's manuals are to be available at the job location and workers are to periodically review them.
-Ensure postings around machinery are current and include appropriate warnings.
-Ensure operators are trained on the proper use of shop tools prior to allowing operation, and require periodic review.
-Hand-holding items against the sanding wheel is considered a normal skill of the craft practice, however, use of positioning tools or a more appropriate method, such as clamping the blade to the work table and using a hand-held grinder are recommended.
The Safety Representative conducted a briefing with all employees who utilize the machine on the correct and safe use of the State Disc Sander and provided alternatives.

<h2 style="white-space:nowrap">JLab Preventive Measures</h2>
<pre>Jefferson Lab considers machine tools to be inherently risky having an unmitigated Risk Code 3. An Operational Safety Procedure (OSP) is written for each machine tool and read by machine operators prior to use. These OSPs comply with and often include the operator's manual. Each machine has a designated owner who is responsible for ensuring that only authorized and appropriately trained individuals use the machine.</pre>
