<pre>On January 12, 2021, a researcher at the National Renewable Energy Laboratory (NREL) in Golden, Colorado, opened an approximately 20-year old, explosion-proof flammable chemical storage refrigerator/freezer (unit) and noticed a mild ammoniacal smell. The researcher then opened the upper freezer section where the smell was stronger to check its condition. It was then determined that the unit was not functioning properly. </pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On January 12, 2021, an NREL researcher opened an approximately 20-year old, explosion-proof, flammable chemical storage refrigerator/freezer (unit), twice to retrieve samples. The researcher noticed an ammoniacal smell coming from the unit that was stronger in the freezer section, so opened it. Ice was still present, but had thawed enough to fall from the top of the freezer compartment, this confirmed to the researcher that the unit was not functioning properly.
The researcher sent an email to the designated area representative telling them the unit was not working properly, and that the chemicals stored in the unit were producing a strong odor.
A group email, was sent notifying lab users of the issue, indicating there was concern about the chemical odor and requesting respiratory protection qualified workers to investigate the issue.
SERF Environment, Safety and Health Points of Contact were contacted. Signage was posted on the unit and access to the lab was restricted.

<h2 style="white-space:nowrap">Analysis</h2>
<pre>The unit was shared by multiple research groups and primarily used for storing solid and liquid research samples. Specific information regarding the types of samples and the potential hazards presented by those samples, was not immediately known. Inventory collection efforts took most of the day to complete.
ESH POCs, hazardous waste management staff, research line management, work group representatives, and Chemical Response Team Lead met to discuss the potential hazards and a path forward for safely accessing the unit and investigating the state of its contents. Ultimately it was determined that there was only one known collection in the freezer that was temperature sensitive. Many of the freezer samples classified as hazardous were being stored at a low temperature primarily to preserve chemicals or extend sample life.
Ultimately ESH safety and waste management staff - wearing full-face respirators with organic vapor cartridges - placed dry ice in the freezer to keep the temperature down until the hazardous material could be safely removed for disposal the next day.

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Utilize a preventive maintenance program or ensure staff monitor aging equipment closer than newer equipment, especially those pieces of equipment that are serving a safety/hazard mitigation function
Discussed the need to evaluate the potential cost impact of running equipment to failure rather than proactively replacing aging equipment
Consolidate highly sensitive chemicals that should be monitored more closely, rather than having them dispersed in numerous locations where they are more difficult to track.
Require and enforce proper labeling and maintenance of research samples. NREL has a robust chemical safety program, which includes labeling requirements for chemicals/samples. Significant labor was utilized trying to identify the chemicals that presented a hazard and delayed the development of an action plan.
Maintain emergency contact signage. Although the unit was posted with emergency contact signage, it was not accurate. NREL does not govern Emergency Contact signage and does not prescribed guidance for updates, or clear roles and responsibilities. Contact information has been determined to be unreliable and outdated across the site.
