<pre>A 30 year old in-house built mobile vacuum pumping "turbocart," was plugged into a receptacle whose ground connection was found to lack adequate tension. When touching the equipment, two workers received minor electric shocks.

<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>The turbocart had been assembled 30 years ago and was last inspected 10 years ago. When Worker 1 touched the cart, they thought it was a burr on the metal edge so did not respond to it. When Worker 2 touched the cart, they felt a jolt in their right forearm. It was then the workers realized there was an electrical issue.
When the workers unplugged the cart, they noticed that the receptacle around the cart's plug was loose and they surmised that the outlet grounding was inadequate. They then covered the outlet with tape and de-energized and locked out the circuit.
The workers contacted their supervisor and were directed to report to the Occupational Medicine Clinic (OMC), where they were evaluated, and returned to work without restriction.

<h2 style="white-space:nowrap">Analysis</h2>
<pre>The receptacle was removed from the wall and inspected. Resistance measurements confirmed that the receptacle's ground connection provided only slight tension on the cart plug's inserted ground pin. The design of the receptacle's ground connection was such that there was only a single blade to contact with an inserted ground pin. Thus, the receptacle failed to provide an adequate ground connection.
The turbocart was also inspected and found to have a floating voltage on the cart chassis. This contributed to the electrical shock experienced by two workers.

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>- Replace the 36 incident-type receptacles in the facility with new receptacles.
- Inspect the receptacles that were removed. Document and report result(s) to the Electrical Safety Advisory Committee to determine if further action is required.
- Submit each of the seven HITL turbocarts into the equipment acceptance program for inspection. Remove any from service if it is determined that it is no longer viable.
- Conduct an independent extent of condition review on other receptacles that may require replacement. Present the results of the corrective action effectiveness review to the BNL Institutional Risk Management Committee.
