<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>Employees working in the Test Lab High Bay area were unable to locate the source of an unfamiliar odor. They contacted the TJNAF Fire Marshall, who requested the Newport News Fire Department (NNFD) respond under non-emergency protocol to inspect the area. The source of the odor was found to be coming from test materials being heated in a thermal heater. No injuries resulted as a consequence of the event.</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On March 28, 2022 at around 2:30pm, personnel from ESH and Experimental Physics were conducting a routine walk-through of the Test Lab High Bay (Bldg. 58) when they detected an uncharacteristic odor. After the walk-through, the ESH employee consulted with the Industrial Hygiene (IH) Subject Matter Expert(SME) regarding the odor. Based on the description provided, the IH SME determined that there was no immediate urgency as the ESH employee did not present any indication that the odor was causing symptoms of irritation. It was agreed that there would be a follow-up inspection of the area the following morning.
An hour or so later, an Engineering Division employee entered the area and made comments to another employee about the odor. Both employees searched for the source of the odor, which appeared most noticeably near the Test Lab North Annex, however due to posted access restrictions they were unable to completely investigate the area to locate the source.
After about a half an hour of searching the employees contacted the TJNAF Fire Marshall. The Fire Marshal was unable to contact IH SME, so requested the Newport News Fire Department (NNFD) to respond under non-emergency protocol to inspect the area. The area representative from the Physics Department was also notified and he immediately came over to assist.

<h2 style="white-space:nowrap">Analysis</h2>
<pre>The NNFD searched and found that the odor as coming from test materials being heated in a thermal heater in a condoned off area. Physics Division personnel were conducting a prototype thermal test using an assembly containing heat-trace tape. The Physics Division representative powered off the thermal heater and the NNFD left the scene.
The next morning, the IH SME was informed that the NNFD had responded on-site. The source of the odor had been discovered and mitigated.

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>The configuration of the Task List Form did not prompt an author to disclosure the potential for emissions/fumes, and "Poor Ventilation" is only addressed in the context of confined space. The Task List Form will be re-evaluated to ensure all reasonably anticipated hazards are more comprehensively addressed.
An ESH hazard review of the Task List is performed by a single source, whose role is to screen and seek input from other ESH SMEs, when needed. This review process will be re-evaluated for effectiveness.
The Task List form was posted in the vicinity of the test, as required, but was not readily visible to effected employees due to the configuration of the area and the roped-off boundaries prohibiting pedestrian entry. Area posting requirements will be re-evaluated to ensure visibility to all effected personnel.
