<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>On Wednesday August 10, 2022, at about 2:00pm while performing the end of the day activities, a skid-steer operator and labor were placing road plates over an open trench. The skid-steer operator was using the skid-steer to push the plates together. (See Photo 1 and 2)

The laborer gestured with a hand signal, requesting the skid-steer operator to stop, but before receiving acknowledgement they proceeded to put a hand between the trench plates to remove some debris.
The skid-steer operator was unable to see the hand signal and continued pushing the plates into place. This resulted in the crushing of multiple fingers between the plates.

<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>The skid-steer operator immediately stopped work and notified the Foreman who radioed the project safety officer. The safety officer performed first aid and called 911. Emergency Medical Services arrived and administered additional first aid. The worker was transported to an offsite medical facility for treatment.</pre>
<h2 style="white-space:nowrap">Analysis</h2>
<pre>- Task complacency may have been a factor as the same trench plate movement activity had been performed daily over several previous weeks without incident.
- Neither the skid-steer operator nor the laborer had confirmed a method of communication (e.g. eye contact, hand gestures, or verbal) nor how to confirm messages were received.
- Work activity occurred towards the end of the work shift.
</pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>First Aid was administered.
911 called.
The site was secured and an investigation initiated.
Written statements were collected from those involved or on scene at the time of the incident.
