<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>On August 10, 2022, two Jefferson Lab Facilities Maintenance and Logistics (FML) technicians were working on a rooftop air conditioning unit on the Guard House (Bldg 51). Two Thomas Jefferson Site Office (TJSO) personnel were walking by and determined that the fall protection configuration worn by one of the technicians would not prevent the individual from striking a lower surface in the event of a fall. The TJSO personnel got the attention of the technicians and suspending the work. There were no injuries or damage to property because of this event.</pre>
<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>On August 10, 2022 two TJSO personnel approached the Jefferson Lab Guard House when they observed two FML technicians on the roof (photo attached), performing work on an air conditioning unit. Both workers had donned full body harnesses; one appeared to be tethered (anchorage not identified) via a self-retracting lifeline (SRL) and the other with a lanyard having a de-acceleration device. As one technician knelt in front of the unit his heels were seen protruding above a small (+/- 4") parapet wall. The TJSO personnel requested that the workers suspend the activity, to which they complied.

The workers descended, and the TJSO personnel requested to see the work control documentation (WCD) to which the reply was that the FML Technicians did not have a WCD as the work was considered ?urgent/emergency? and had been assigned via text message from the supervisor. The fall protection configuration was discussed and it was concluded that it would not prevent them from striking the lower level during a fall event The TJSO personnel proceeded to the FML Division Safety Officer?s (DSO) office to advise him of their concerns and of the resulting work suspension.

<h2 style="white-space:nowrap">Analysis</h2>
<pre>The FML Supervisor assigned the work as ?urgent/emergency.? This category of work does not require that the Work Planning, Control and Authorization (WPC&A) process be followed.
Lacking the Work Planning, Control and Authorization (WPC&A) process, no hazard analysis or associated safety procedure (OSP, TOSP, or FPP) was developed.
</pre>
<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>A written communication will be provided to all FML Division personnel addressing the Division's use the formal WPC&A process, and that an internal classification of assigning work as "urgent" or "emergency" does not exempt FML personnel from the use of the process.
Training on the WPC&A process will be provided to all FML personnel, by pertinent ESH resources, to ensure comprehensive proficiency.
FML managers and supervisors will intensify their daily oversight responsibilities for their workers engaged in activities where pre-mitigation hazards are greater than two (RC > 2).
Add to the Active Fall Protection training curriculum a supervised practical demonstration, to verify learned/retained knowledge and proficient use of active fall protection.
FML will analyze all elevated working surfaces and determine whether each could accommodate passive fall protection. The analysis will include justification for any conclusion against installing this protection.
