<h2 style="white-space:nowrap">Statement of Lessons Learned</h2>
<pre>On June 8, 2022, while installing insulation and metal roof panels, a construction Ironworker fell sideways through an opening physically smaller than his body. The Ironworker was wearing the appropriate personal fall protection system (i.e., self-retracting lanyard) (See Photo 1 for an example). During work activities they bent forward to straighten some insulation dislodged by wind. The protection system suddenly and unexpectantly engaged causing the Ironworker to lose their footing and fall sideways through an opening in the roof. The Ironworker was subsequently lowered to the ground by a co-worker from inside the building via an aerial lift and transported to the site's Occupational Medicine for a medical evaluation. They were then released and returned to work with restriction(s). There was no impact to the facility, however construction work on the building was stopped due to the event investigation.

<h2 style="white-space:nowrap">Discussion of Activities</h2>
<pre>During a review of this event, it was identified that after the Ironworker fell and was rescued, the construction subcontractor did not abide by contractual obligations. The subcontractor should have immediately stopped work and make appropriate notifications that would allow for Industrial Safety and Construction Management to respond and investigate the event. Instead, workers were allowed to go back on to the roof, take pictures. and patch- the hole that had been made in the insulation.
The subcontractor also failed to notify the Hanford Fire Department Rescue Team per the Fall Protection Work Permit (FPWP), Section 7, Rescue Plan to ensure the safest possible rescue.
The subcontractor also was not able to provide evidence that annual inspections, performed by a competent person, had been completed on either the self-retracting lanyard nor the standing seam roof clamp.

<h2 style="white-space:nowrap">Analysis</h2>
<pre>The Ironworker was wearing appropriate Personal Protective Equipment (PPE) including an approved Personal Fall Arrest System (harness, self-retracting lanyard, and standing seam roof anchor), safety glasses, work gloves, work boots, high visibility shirt, and hard hat.
The self-retracting lanyard is designed to react similar to a tape measure, allowing the wearer to adjust their location (distance) from the anchor point using slow and fluid movements. Apparently, the worker?s bending forward movement was fast enough to cause the SRL to engage.

<h2 style="white-space:nowrap">Recommended Actions</h2>
<pre>Elevated work across the site was paused for a work crew briefing about the event. This included detailed explanation of the job specific Fall Protection Work Permit process; proper use of Personal Protective Equipment (PPE) specifically reinforcing the concept of utilizing smooth, fluid movement when using self-retracting lanyard; and the importance of timely reporting of injuries and events.
The Ironworker?s fall protection harness and lanyard were segregated for inspection/destruction.
The Lessons Learned was assigned as required reading to all personal fall arrest system authorized users, Field Work Supervisors, Fall Protection Competent Persons, Fall Protection Qualified Persons, and Fall Protection Spotters.
The Lessons Learned is required reading in future Work Planning and Work Instruction Development packages that contain the use of personal fall arrest systems.
A meeting was conducted with the construction subcontractor where set expectations were reviewed regarding to the conditions found that were adverse to quality.
