[Pansophy] [New Logentry] Updated TRAVELER_FORM.cfm for SNs and SNTYPEs

The auto fill for NCRs and D3s was intermittent. This was due to the fact that 
the SN and SNType were not being passed to all pages.
Each place a call to TRAVELER_FORM is made the SN and SNTYPE was added to the 
URL. This has corrected the problem.
