Hi Anne,
a recommendation from the "lesson learned" would be for the Inventory Group to make a list with the SN of the end-groups as they are getting packed for shipment and add that list to the Shipping Authorization form: this would allow us to be sure what SNs the end-groups have when they leave the lab.
I checked the Shipping Authorization form (#107555) for the shipment of the end-groups that RI is currently working with (approved 10/22/2020) and I did not find any information about the end-group SNs.

as we discussed, I'm contacting RI to see if they can help tracing the end-groups SNs.
I personally do not have a preference on how the end-groups are serialized. If there is a procedure on how to assign SNs, I guess it should be followed. I see this as a QC issue and I will let the QC group handle it, with the only hope that if similar issues are found upon receipt of the next batch of cavities from RI, they would be dealt with faster than ~3 months.

It looks like the we are fairly certain of the SN that was used – is likely CEBAF -008 based on the thin flange and the welded on “threaded button” shown in the picture, both of which are unique features as explained in Aaron’s note below
Aaron pointed out also that RI also has put a SN on the FPC, I am wondering if they also have a list where they recorded against their SN all of our SN on the old end groups ? If so this would be an excellent cross reference to have available for future
I have also asked the Pansophy Team if they can generate a list of the SN – what is available and what has been used – but we likely won’t learn more than what Aaron has already uncovered by using Scotts List that is attached
To prevent these problems in future, with the many SN on the old FPCs & HOM of C75 cavities I am wondering if we need to add a “C75” or another designator beside the SN that is to be used (so it is clear which SN should not be used)
Anyway please let us know your comments

The notes from the inspection excelsheet prior to shipping to RI leads me to believe 008 is the correct number. The Fpc flange is very thin which is unique, only a few cavities have this issue. Also, the picture in excelsheet matches before and after RI showing a unique feature (welded block with threaded hole) on fpc end group.

C75-RI-011 is in the tuning room last I saw. It was on a cart with a yellow hold tag on a flange cover.
If it helps, Gigi and I were able to see IA-008 scratched out on the FPC and CEBAF aim the HOMs.

As of Tuesday according to the schedule, it looks like you had C75-RI-011 in chemistry , do you know where this cavity has gone and is it accessible ? After Feb 9th it disappears from the schedule so I am not sure where it is now
Gigi would like an investigation to find out what is the SN for the end group , so the next time it is available to us we would like to be able to see it again

I understand you have a list of the Serial numbers for the end groups that we originally cut off – If you have this can you please send it to us
We need to try and figure out what the correct serial number was for this end group before it was scratched out (if possible )
Unfortunately it seems that the serial numbers were not available in the drop downs original traveler when it was first inspected back in November , subsequently it seems that the SN was scratched out on the part – so as a result we may have lost traceability on what end group ended up in this cavity
At this point we can only do our best to try and figure out by process of elimination which end group was used

Hi all,
there seems to have been a chain of unfortunate events that has prevented us to associate the end-groups serial numbers to the proper C75 cavity SN:
* During inspection of the cavities from which the end-groups were going to be cut, it was agreed to serialize the end-groups with "EG-cavity SN" and this is how they were recorded in the travelers C20-CAV-INSP-FPCEG and C20-CAV-INSP-HOMEG
* When the cavities arrived from RI and were inspected by CMM group, those end-group SNs were not available in the drop-down menu for the traveler C75-CAV-INSP so the end-group SNs were left undefined in the cavity inspection traveler
* After the end-groups SNs were added to the drop-down menus, I was informed yesterday that some of the EG SNs were "scratched out" and not visible.
* This morning, I asked Ashley to check the thickness of the FPC flange on cavity C75-RI-011 and she sent me the pictures below which indeed show that the end-group SNs were scratched out and replaced by some other nomenclature
How did the end-groups got the SNs scratched out and re-serialized? How was it decided?
Thanks for your help and for a) suggesting a fix to the issue, b) fix the issue, c) suggest ways to prevent similar issue to occur again.

I took thickness at the 4 corners so as not to scratch the flange sealing surface. Measurements are in inches: 0.3005, 0.3490, 0.3270, 0.3280
As for the SN#s, some are scratched out.
FPC says FLG#3 and MM#2 (and cavity number in two places)
