Hey Allen,
I clicked on the link this morning to see if I could approve the new document but there's nowhere to click for approval and nothing pending in "My Tasks" in My Docushare section either.  Not sure if this helps at all.

Since no one has approved the document yet, I’ve terminated the routing and will change Tiffany’s name out for yours (and fix the revision number since I just noticed that too). The actual author of a traveler has to be in the Author approval slot. We can swap one of the other reviewers out for Tiffany, or have her unofficially evaluate the traveler, but I’ll have to swap you in as the Author. Let me know how you want to proceed, and I’ll make the changes before routing.

   I am okay leaving this traveler this way as I just wrote the revision, Tiffany knows more about the process, and I would prefer having her double check the revision. If that is allowed?

Should this be routed to Adam for signature instead of me?  Looks like maybe the author approval line just didn’t get update to replace my name with Adam’s?  If I I need to be “author approval” signature, then let me know and I can review the document.
