Adam,

I’m very sorry, but I have to send this back to you again. I didn’t notice it before, but you’ve split up what was the first page and removed some fields. In doing so, you took out the SRF field; one that is required on the first page of a traveler. In addition, a timestamp, SN field of some sort, and comment box are recommended. These can be found on the second page. To fix this, our best options would be to either recombine the two pages, or add the necessary fields to the new first page. How would you like to proceed?
