It does state in the document form what the training is. It's not exactly the same training name, but it's close enough that they should be able to figure it out. I still can if you want though.

Do you think the slide set or link should go with the email if they want to see which class they are signing for?

Subject: Acknowledgement Form email
John,
I was going to send an email out to the people we're getting to sign the Acknowledgement Forms. I was wondering what your thoughts were and if you had any changes you wanted to make.
Good afternoon, all,
You may notice, or have already noticed, an unusual approval routing in your inbox. As part of our ISO effort, we are using a new Acknowledgement Form to keep records of training. This form is sent out to you, your supervisor, and an authorized evaluator for the training you completed to confirm that you now have the necessary skills. If you have never had to approve a routing, simply login to your docushare account (which should be your same login information for the JLab website), then follow the link in the email to review and approve the document. If you have any questions, reach out to your supervisor or us at pansophy at jlab.org<mailto:pansophy at jlab.org>
Thank you for your time,
Allen Samuels
