By the way, I'm surprised you do not use PDF format for these documents to have a little more confidence they are not modified when somebody reads or prints them.

Thanks for the clarification, Roger. The procedure has been routed for approvals once more.
I think I know what happened with the header and footer. For some reason if you delete the form approval and the section break for it, you'll also delete the header and footer. Not sure why, it just happens. I just copied the information from your version and put it in the version I made that still had them. Everything should be fine, but if you notice anything amiss, please let me know.

It went into DocuShare just fine. Unfortunately something happened and the headers and footers disappeared, so I'll fix those up for your.
I notice that Tony is back listed as the project representative. Did something change with the plan for the third approver?
