[Pansophy] NEW Cavity Types Page

All,

This summer my student, Gabriel, worked on updating the Cavity Types view and update pages. We now have them in production and I would like to kindly ask you all to review and make note of any necessary changes or let me know if you think it is all good to go.

The page is currently operating on a direct copy of the original database which I would like to eventually make the working database. I know there are some cavity types in a spreadsheet somewhere that may need to be added to the database, which I can do for you.
