I have no idea, I select everything on the form page and press delete and then go below the approvals box and press delete with the paragraph marks visible so I can see what I'm doing and it just works.  Lucky I guess?

Looks good! How did you get the header and footer to stay after deleting the page break between the form and procedure approvals?
