Naeem,

For some reason if you delete the section break right before the form approval page, it removes the header and footer. We don't know why it does this, our only solution is to not delete it and edit the extra page out after it's been converted to a PDF.
At this point your options are to either hit undo until you undo the deletion and it should restore the header and footer, or open the template again and copy all your work.

Hi Allen and Matt,

I was using the procedure template to write the attached procedure and I somehow lost the headers and footers. Is there some way of getting them back without having to copy and paste into a new document?
