We will clear the database tables (reset) before releasing into production for all people.

This is great!  Thoughts below.

General Information section:
-Replace "Assembly SN" with "Drawing Number" and include a file upload in addition to text box.
-Quantity should be an integer-only field, add a second comment box for the user to enter specific serial numbers (if known - would be great if this could be integrated into the overall pansophy/primes serial number system)
-Replace "Scientist Name" with "PI/Engineer" - having this be a user dropdown would be nice but may not be feasible given that not all requestors will be SRF.  Maybe include a second line for contact email?
-Move "Return Location" to the "Completion Information" section

Joint Information and Base Material Information can be combined under "Joint Information", with the base material information going first.

-"Joint Type" should be prepopulated with "Groove", "Step Groove", "Lap", "Fillet", "Corner", "Other-Add details to comments"
-Replace "Joint Penetration" with "Penetration Depth(mm)" with a number input (decimals allowed) instead of radio boxes.
-Can remove the fixture image and sketch upload sections

-Move "Request Description" to it's own section after base material and before completion with a comment box, would also be good to have a few upload slots here for procedures, additional drawings, etc.

It would make sense to have a section on inspection requirements, however since this is an EBW request and inspection is generally outside the scope of EBW, it may cause confusion.  Possibly adding a comment section to the "Request Description" area that says "Inspection Requirements" with a note at the end saying "All EBW welds receive a cursory visual inspection, additional inspection may require coordination with additional workcenters and/or outside vendors to be handled by the PI/Engineer"

When you select OTHER, then a box will be presented for you to input your selection.
That when then be added to the dropdown for future use.

I notice the drop down menus only have the option of "-0-" or "Other" and no space to write what might be other, suggestion to add a comment box or more options to the menus.

We are working to complete the EBW service request the interns started over the summer. This system will help track requested work. With the bulk of the work now completed we would ask you to look over the page and provide any suggestions. We hope to demonstrate the functionality to you shortly.
