For the Appendix with the data from the tech note, link to the original document from DocuShare. As far as the excel sheet that is acting as a form/worksheet, I am going to defer to @Valerie Bookwalter<mailto:bookwalt at jlab.org> on that since she is the process owner for Document Management

Could you maybe provide some more information so I can better help?
Are you asking where to put the data? Is this for CM or CAV?
If you're looking for what traveler it goes in, this question might be better suited for the people you have CC'd on your service request and this email. Or if you're looking for where to store them in DocuShare, maybe this directory? https://jlabdoc.jlab.org/docushare/dsweb/View/Collection-51703 Again, probably a question for others.
