[Sbs_gems] [New Logentry] UV Layer#2 APV work complete

We first replaced the "bad" APV card which wouldn't let CODA to start running. However, that did not fix the issue. So after a series of tests, we decided to switch the 3 slot back plane with a 4 slot back plane (because we don't have any spare good 3 slot back planes). After the back plane switch, the issue went away and now CODA is running fine with that APV card which had been removed from the DAQ for the past 4 days or so. 

A new GEM pedestal will be uploaded into the DAQ and database soon.
