I believe they followed the following steps:
1) bunch the fibers together, cut and polish, then sputter (add reflective plating);
(pictures with filenames starting with 1 through 6).
2) hold the fibers in shape (see http://hallaweb.jlab.org/experiment/PVDIS/SoLID/EC/meetings/20150505/WSU/7-shaping.JPG)
3) insert all fibers into the module, with the sputtered end aligned with the bottom of the module;
4) glue the other end of fiber (that side that go to the PMT) together into a small circular holder with holes,

What I do not know is what they did between steps 8 and 9. They must have cut and polished the fibers after gluing them to the small circular holder, then coupled the holder to the PMT.
For our test here, we managed to 3D print some fiber<-> PMT coupler. They are simply plastic caps with 100 holes on them. Vince tried to insert fibers into it for the shashlyk layer test last week before he went to JLab. He could add what he thinks on how these coupler would work. The 3D printing has its own problem (holes not completely open, etc,), so I may ask the machine shop here to make one from black plastic (delrin).

Sorry for my worse link.
About the fiber. If we hold one bunch of fiber and polish them together, it conviences to polish. But it will be difficult to insert all of them together into the detector, and some fiber can't reach the end of the detector if we cut them at same length. Also we should straighten the fiber, so it will be easy to instert. But how to straighten the fiber?
If we insert the fiber before bounding the fiber, it will be difficult to polish the end which coupled to the PMT.
>From ALICE's document, they divide the fibers into 4 bounch (not 1 bounch) before insert, and the fiber is straight.
