Hello, All:
Yesterday, a worker fell several feet, sustaining a back injury. The
worker used a moveable stair-step device to ascend several feet to a
working position. While descending and preparing to step on the moveable
stair-step, the worker fell.
Moveable stair-step devices should not be used to ascend to or descend
from an elevated work space. A moveable stair-step is designed to be
used with the weight of the user constantly applied over the center of
the device so that it does not move.
Make sure that moveable stair-steps are maintained in working order and
are used properly and in accordance with the manufacturer's
instructions. Remember, when transitioning to an elevated work position,
do not use a moveable device of any kind unless it is positively secured
against movement. See the ES&H Manual Chapter 6132 Appendix T1, Ladders,
Section 4.3.2.3 Mobile Ladder Stands and Mobile Ladder Stand Platforms
for important information.
This Lessons Learned notice is being shared with the Jefferson Lab
community to make everyone more aware of stair-step hazards and
requirements in order to prevent similar mishaps in the future.
Pictured below is an example of a stair-step device.

Thank you for your attention to these important safety matters.
