Ladies and Gentlemen:

Phil Mutton, who has been the Accelerator Division Safety Officer (DSO) since 2007, is preparing to retire.  Phil has done an excellent job as DSO, particularly in the difficult preparation of the new Final Safety Assessment Document (FSAD), and deserves all the credit for the successful completion of this task.  As the first step in the retirement process, Phil has asked to start working half-time starting on June 1, and I am pleased to support this request.  Phil and I have agreed that he will work on Mondays, Tuesdays, and Wednesday mornings.

In order to ensure that safety in the Division remains a top priority, I have asked Harry Fanning to become the Accelerator Division Deputy DSO, with the intention that he will eventually take over as DSO when Phil finally retires.

Over the past ten years, Harry has been involved with Jefferson Lab’s safety program on many different levels.  Aside from being Phil’s backup and standing in as Acting Accelerator Division Safety Officer on a number of occasions, he has been the Accelerator Operations Safety Coordinator since 2007 and a Safety Warden since 2003.  Harry has been instrumental in helping to form JLab's Work Planning and Control process along with assisting with the review of our Integrated Safety Management (ISM) system and the Office of Health, Safety and Security (HSS) reviews.  He has been lead investigator on multiple incident and notable event investigations and has assisted in several key program reviews, such as the RadCon Peer Review, CAP Review, Accelerator Safety Review, ISM Effectiveness Review and DOE Oak Ridge Quality Assurance Reviews.

Harry has also helped to develop several safety-related training programs, such as the Accelerator Tunnel Awareness Training, Work Planning and Control and the communication of ES&H Manual changes.  He is the ES&H Manual and appendices author for chapters 3210, 3310, 3320 and 3320, which are all related to the Work Control Planning and Control processes and Work Control Documentation. Harry is a dedicated JLab employee with a very strong work ethic and is committed to excellence. His contributions have positively impacted both the MCC Operations group and the JLab safety environment.

Tina Menefee will continue in her role as the ESH&Q Liaison to the Accelerator Division, a role she carries out in an exemplary fashion, in addition to her responsibility as the JLab Emergency Manager.
