Dear Colleagues,

On Tuesday, Sept. 28

announced a new health and safety requirement that all employees, Users,
subcontractors and visitors must be screened for COVID-19 before coming
on-site if they have not disclosed whether they have received the COVID-19
vaccination.

We will implement this new requirement, with the first testing day on
Wednesday, Nov. 3. We are providing this two-week notice to allow ample
time for anyone who is fully vaccinated to submit their vaccination record
to the Occupational Medicine Clinic (OMC) if they have not done so
already.

JSA Employees
COVID-19 testing is not required for JSA employees who have submitted a
copy of their COVID-19 vaccination record to OMC using the secure dropbox.
If you are not sure if your vaccination record has been captured, you can
verify it here (https://misportal.jlab.org/jlist/covid_statuses/self) .

If your vaccination record has been captured, you have no further action
to take and may continue to have site access. For those who have not
submitted their completed vaccination record, please do so now.

On Saturday, Nov. 6 at 6 a.m., any employee who has not submitted a
COVID-19 vaccination record or a COVID-19 test result will be designated
as undisclosed. Undisclosed employees may only come on-site if they
participate in our weekly on-site COVID-19 testing program and have a
negative test result.

To participate in the weekly on-site COVID-19 testing program, undisclosed
employees are required to schedule a COVID-19 test appointment the week
prior to working on-site. We will offer COVID-19 testing every Wednesday,
starting on Wednesday, Nov. 3. Test results should be available the
following Thursday or Friday.

The testing location will be in the Support Services Center (SSC, Bldg.
28) OMC with appointments from 9 a.m.-noon and 1-4 p.m. Tests will be by
appointment only to maintain safe spacing of staff. You will need to
register on the testing vendor’s Aura

need to enter your personal demographic information and schedule your
appointment for weekly testing. This registration/appointment system will
be available by close of business on Friday, Oct. 22. Tests typically take
3-5 minutes. Please check in with SSC lobby at your appointment time. You
will then be directed to the OMC to provide your sample. To sign up for

The test is an FDA-approved polymerase chain reaction (PCR) test
administered by saliva collection. The tests are 98% accurate. Results
usually return within 48 hours. The test will be self-administered under
the observation of the OMC staff who will ensure proper chain of custody.

Tested employees and the OMC will receive test results by email
simultaneously. The OMC will report negative test results of undisclosed
employees to JSA Security each Saturday and JSA Security will reactivate
badge access through the Saturday following the week of the test.
Undisclosed employees will be required to repeat testing each week, or
until two weeks after they are considered fully vaccinated and have
submitted a completed vaccination record to the OMC. Undisclosed employees
who test positive for COVID-19 will be medically restricted from coming
on-site until cleared to return. Undisclosed employees who do not
participate in weekly testing or miss their test appointment will be
unable to work on-site during the following week or until they have a
negative test result.

There will be no cost to JSA employees for COVID-19 testing administered
at the OMC. Any remaining testing appointment times will be open for
vaccinated employees each Wednesday on a first come-first served basis
after all undisclosed employees requesting site access for the following
week are scheduled. You will need to use the scheduling app

This testing program for undisclosed employees will be in effect until
Wednesday, Dec. 8; after that date, all employees must be fully vaccinated
or have an approved accommodation as stated in the Vaccination Requirement
Deadlines message

Thursday, Oct. 7. After Wednesday, Dec. 8, only employees with approved
accommodations will be required to participate in weekly testing to
maintain site access privileges.

Please remember that vaccination and a negative test result are not
substitutes for employee responsibility to perform the Daily Health
Self-Certification

and report to OMC staff if you have symptoms or meet the reporting
requirements.

JSA Subcontractors
JSA has informed all companies who have staff working on-site of the
requirement to ensure all of their staff who physically work on-site at
Jefferson Lab must be fully vaccinated or have a negative COVID-19 test
result within three days of any day the employee works on-site and provide
that status to the lab weekly.

This requirement does not apply to delivery drivers who do not enter our
buildings or approach within six feet of our staff.

Users with Current Site Access Privileges
Users with current site access privileges must submit a copy of their
vaccination record to the OMC using the secure drop box following
instructions on the COVID-19 portal

We will designate Users who have not submitted a COVID-19 vaccination
record to the OMC as undisclosed. Undisclosed Users will have their badge
access disabled and may not come on-site until they provide negative test
results. To come on-site, undisclosed Users will be required to submit a
negative COVID-19 test result taken within three days of their next
on-site arrival at Jefferson Lab and submit a copy to OMC staff using the
secure drop box. JSA Security will reactivate badge access for these Users
for seven days. Undisclosed Users will be required to submit test results
each week or until they are considered fully vaccinated to OMC to retain
site access privileges.

Users without Current Site Access Privileges
Users who need to come on-site and do not have current site access
privileges must submit a site access registration request through EARS as
described in Section 2.2.2 of the TJNAF COVID-19 Workplace Safety Plan

and a copy of their vaccination record to OMC staff using the secure drop

Once site access registration is complete, Users who have submitted a
completed vaccination record to OMC will have their site access activated,
or reactivated if they already have a JSA badge. Users who do not submit a
COVID-19 vaccination record to the OMC will be designated as undisclosed.
To come on-site, undisclosed Users will be required to submit a negative
COVID-19 test result taken within three days of their next on-site arrival
at Jefferson Lab to OMC using the secure drop box. JSA Security will
reactivate badge access for these Users for seven days. Undisclosed Users
will be required to submit test results each week or until they are
considered fully vaccinated to OMC to retain site access privileges.

Visitors
Visitors are required to check in at either the CEBAF Center lobby or SSC
lobby to show Jefferson Lab Security a copy of a completed COVID-19
vaccination record or a negative COVID-19 test result taken within three
days of arrival to be granted access to Jefferson Lab. Hosts should make
sure to remind visitors of these requirements before they come to
Jefferson Lab.

COVID-19 Testing for Subcontractors, Users and Visitors
JSA can only provide COVID-19 testing for employees. Unvaccinated
subcontractors, Users and visitors are required to secure their own tests.

Full Vaccination is Required
As a reminder, you are considered fully vaccinated 14 days after the
second dose of the two-dose vaccine or the first does of the one-dose
vaccine. There is no site access requirement for booster shots at this
time. We encourage employees to follow CDC guidance and get booster shots
when it is appropriate for their health status and time since initial
vaccination.

While the vaccines are highly effective at preventing severe disease,
effectiveness may decline over time and vaccinated individuals can be
asymptomatic and transmit the virus. With this in mind, Jefferson Lab will
continue to maintain additional controls as outlined in the TJNAF COVID-19
Workplace Safety Plan
(https://www.jlab.org/sites/default/files/TJNAF%20COVID-19%20Workplace%20Safety%20Plan%20Rev%207final%209-2-21.pdf)
for Medical Condition (MEDCON) 4, such as wearing of face masks, limiting
site density through telework for all staff not required on-site, daily
health self-certification checks and other measures until it is safe to
relax these controls.

Exceptions
Undisclosed employees who are required to work on-site but are unable to
participate in COVID-19 testing for medical or religious reasons may seek
an accommodation through Human Resources. Accommodation requests must
describe the basis for an accommodation and if the basis is medical, the
request must include a statement from a Doctor of Medicine (MD) or Doctor
of Osteopathic Medicine (DO) licensed to practice in the Commonwealth of
Virginia describing the medical condition that prevents an unvaccinated
employee from submitting a saliva sample for COVID-19 testing.

There is no exemption from site access requirements for employees who
normally work remotely but need to make a casual visit to the Jefferson
Lab campus or their office.

Requests for emergency exemptions for subcontractors or Users requiring
site access should be directed to the ES&H Director

Emergency exemption requests must describe the extenuating circumstances
that prevented the subcontractor or User from meeting the site access
requirements and the impact to the Jefferson Lab mission if the emergency
exemption is not granted. The ES&H Director will grant emergency exemption
requests when the circumstances are unforeseeable or unavoidable, the
mission impacts are significant, and other administrative controls are
available to protect Jefferson Lab employees from potential infection by
an undisclosed and untested subcontractor or User while they are on-site.

Failure to Meet Required Health and Safety Standards
Undisclosed employees who are required to work on-site, but do not
participate in weekly testing or are not granted a testing accommodation,
and then fail to report to work as scheduled for three days without proper
notification, absent extenuating circumstances, shall be considered as
having abandoned their job which constitutes grounds for immediate
separation from JSA employment. See Administrative Policy 208.01
