[Cuga] **Reminder**Visitor Insurance Renewal Information- A message from Tara Tyndall

Visitor medical insurance renewal for 2015/2016 begins on October 1, 
2015. New policy information and insurance cards will be available for 
pick-up at the SSC front desk (building 28) beginning Thursday morning, 
October 1, 2015.

Old insurance cards will no longer be accepted at doctor's offices or 
medical facilities after September 30, 2015. Please be sure to review 
all coverage changes and be aware of your insurance responsibilities.

All visitor insurance plans include a MEDCO prescription discount and 
AXA travel assistance services.

If you have a visitor insurance plan through the Lab, please fill out 
the attached HIPPA form as well as the correct enrollment form and bring 
with you to the SSC.
