After we received more information from the investigation, we have expanded the scope of the pause include all hazardous energy control LOTO work. Effective immediately, we are implementing a lockout/tagout (LOTO) pause on all activities requiring hazardous energy control. This includes all sources of hazardous energy, not only electrical energy.
Further, there shall be no work performed on any system that has been locked out during the SAD until we complete a verification of each system’s isolation certificate and isolation points in the field. This full assessment and verification of the isolation points is for all active locks since the start of the scheduled accelerator down period that began on Monday, May 20 and is already underway.

These issues are consistent with a weak safety culture and drive us to take the time to reflect on how we do all our work at the lab, from driving vehicles on-site to complex LOTO work.
Over the past year, while we have made some improvements to our safety posture, there is still work to do. I want to thank everyone who has contributed to these advancements. However, there are still gaps in our processes, competencies, performance and the underlying safety culture that must be urgently addressed.
Over the weekend, a team of members from across the lab will develop a Hazardous Energy Control Pause and Restart Plan in order to establish our path forward. In the interim, no one may perform any LOTO work until they have successfully completed the competency evaluation process led by electrical safety program lead John Riesbeck and members of his team. The evaluations will be performed on a schedule based on priority established by the SAD Oversight Committee and approved by lab leadership. The committee will add additional team members to include all work, not just work related to the SAD.
