    Add EVENTOPEN/EVENTCLOSE to Bank Tools.  Fix up and update primary readout list.
    
    dmaBankTools.h
     - Added macros to open and close event banks of any type.
    
    linuxvme_list.c
     - Removed redundant ACK macros
     - Added vmeCheckMutexHealth to prestart
     - Add rocCleanup call to library destructor
     - Decode TIR/TS event data during asyncTrigger, instead of usrtrig.
     - When copying data from dma buffer to coda buffer, copy over the length and header.
     - Fix bug... tiNeedAck to tirNeedAck
     - Remove doAck()
