[Dcouncil] Possible survey questions

The questions presently on the survey are great but here are some possible additional,questions. 
Ron 

    1. Do you feel that JSA supports your pursuits and ambitions? 
    2. Do you feel you can express your opinion without fear of criticism? 
    3. Have you (or how often do you) take time to get to know your co-workers personal background? 
    4. Do you have the support of your co-workers (or management) concerning diversity or inclusion issues? 
    5. Should the survey include information on age, gender, race, education, years at the Lab, etc.? I know this is sensitive but if we had this information we might be in a better position to understand the survey results. 
    6. Does your work environment encourage identification of abilities to allow people to develop? 
    7. Have you been assigned a mentor?Yes or No/ Do you believe in the mentoring program?
