Hi Yuri,

Info on revising the OSP is located at 
https://www.jlab.org/ehs/ehsmanual/3310T1.htm in section 4.8. It looks 
like you need to rewrite using all info from the old and add in any new 
hazards/mitigations to the revised document and THA, then be sure to 
refer to the old document in the "Document History" section.
