Hello Gianni,
I have attached a diagram that shows the timing of the relays used in our test.
There is, on average, a 100 millisecond delay between the opening of one relay and the closing of another.
During this time the HV is indeed still on.
I understand your point that this may cause a critical error, and to that end we are looking at redesigning our test.
However, this issue of the board disconnection has now been seen during a completely different test that does not use relays, so switching the relays
with the presence of HV on the output cannot be the sole cause.

this may be can be a critic operation, and may be could induce the problem ( High Voltages discharges?), what do you think?
Depending from the grounding, theHV discharges could put in crisis some boards.
We use similar test set up with relays, to test the boards automatically, but we avoid to switch the relays with the presence of HV on the output.
Could you perform a test switching OFF the channels or putting the voltages at zero before switching the relays to verify if the problem persist?

It is not clear to me the sequence value (Vset) in the script you sent, when you say "Test with load where the voltage was ramped up step-wise. There were a series of relays that would quickly cycle through all of the channels and a digital multi-meter would measure the voltage output of the channel." You mean that the relays are switched when there is High voltages on the channels output?

The script shows the voltages we were using for the test. There was no consistency in terms of losing connection to a board at a particular voltage. The disconnected boards never came back on their own, we have to power cycle the crate in order to re-establish communication with whichever board which we lost communication with.
Also, yesterday we experienced a board disconnection during a different test. It was during a ramping test with load (2 mega ohms) using GECO. This is the first time we've had this happen during this kind of test.

When a board or more boards that populated the crate disappear, after a while it re-appear or are not present any more in the crate map? In this case you have to do a power cycle to see again the boards that disappear?
I had a look to the HV Module Test Stand Setup circuit, I would like to know if when you switch the relays, it is present the HV on the channel, if yes at what voltage you perform the test?

I'was able to reproduce the random parameter changes issue.
The issue was not visible with only one board because it is related to the parallelism inside the IOC.
What was happening is that with several boards the EPICS framework (our IOC is based over it) calls parts of our code in parallel and there was an internal structure that was not thread-safe. The results were that some settings were not applied to the right parameter but to onother one (the aren't random parameter changes, The values were right but the parameters were wrong! (i.e., a Trip = 40 were codified as VSet = 40 or a PW = On where codified as RUp = 1).
I fixed the issue and now I'm testing the results and the issues seem to be resolved. I'll do other tests in these days then I'll send you a new HiVocs version within a couple of days.
I don't have news about the other issue (the board disconnection) in all my test I never experienced a board disconnection.

Really strange a so high number of problems on the panel connector, as told from our experience it is more problematic the cable connector, this is the reason that we add the hot glue.
At this point let me know how you wish proceed.
To rework/repair the boards with pins pushed out we have two solutions, you ship the boards to our office in NY, or I could make an intervention in JLAB in spring.
How many boards you have in total?
May be it's worth to have a look to al the boards.

8 times it was the pins for the safety loops, and 4 times is was an arbitrary pin.
All occurrences were on the board R52 connector (male).

are strain relieved, but the safety loop pins are not.

How many pins per board?
If I well understood ( but may be I'm wrong) there are 4 boards with this issue, that is one pin pushed back,and every time is one of the two safety pin loop.
Could you or some one of the group confirm me these numbers?
Or in case I'm wrong could you provide me a report updated to today?

please could you tell me if the pins pushed out are present only on the RADIALL connector installed on the boards (female) or even on the RADIALL connectors installed on the cables ( male).
>From my experience usually in the past the problem appeared only on cable side, this is the reason that we decided to add the hot glue, in order to strengthen the tightness of the pin and prevent a possible escape from the correct position, but this as said only for the cable connector.
The problem on the panel connector is very rare, how many problematic boards you have with pins pushed out?
