[Eng-div] A Message from Henry Robertson: STOP Program Survey for Supervisors and Non-Supervisors

Please fill out this survey completely by close of business tomorrow. 
The input you provide to us will be used for improvements in the 
presentation and program over the long term. We count on your support 
and participation to make the STOP process work for everyone.

This is a reminder that supervisors and non-supervisors who participate 
in Jefferson Lab's STOP (STOPLight) program, or who have done STOP 
observations, or who have been the subject of a STOP observation are 
being asked to complete a survey about the program.

The ESH&Q survey is hosted offsite by Survey Monkey and can be found here:

https://www.surveymonkey.com/s/D5RYS8H

The survey is anonymous, however, at the end of the survey you can 
choose to provide your name and contact information.

The purpose of the survey to assist the lab in improving its safety 
programs.

The online survey consists of 17 questions and should take just a few 
minutes to complete.

You will have until Tuesday (Dec. 13) to complete the survey.
