Attached please find for review "ES&H Manual Chapter 6540 Oxygen 
Deficiency Hazard (ODH) Control Program" and its associated Appendices:

    * T1 Oxygen Deficiency Hazard (ODH) Safety Review
    * T2 ODH Classifications, Engineering and Administrative Control
      Practices, Training, and Medical Exclusions
    * T3 ODH Emergency Response Procedure

The Technical Point-of-Contact for this material is Dick Owen, ODH 
Safety Review for Jefferson Lab, as such he played an integral role in 
the development of these documents. Technical reviews were provided by 
(in alphabetical order) Robert Adams, Engineering; John Kelly, ESH&Q 
Division; Nathaniel Laverdure, Engineering; Kelly Mahoney, Engineering, 
Mathew Wright, Engineering, Jennifer Williams, ESH&Q Division.  Others 
who were given opportunities to provide comments included Jonathan 
Creel, Engineering; Mary Logue, Associate Director-ESH&Q; Will Oren, 
Engineering Division Manager; Henry Robertson, Engineering DSO.  
Comments were incorporated as appropriate.

 

Draft documents were last reviewed in April 2011.  A substantive change 
was requested at that time which included adding the qualifications 
required to be an ODH Analysis Design Authority.  This was done and 
provided within the chapter document and appendix T1.

 

As a reminder, the content has been separated from "ES&H Manual Chapter 
6500 Cryogenic and ODH Safety," as it was recognized that oxygen 
deficiency hazards are associated with more than just cryogenics.

 

It is asked that you provide these documents to affected members of your 
organization for review. Any comments are to be directed to the ES&H 
Manual Editor (MBailey at jlab.org <mailto:MBailey at jlab.org>) by October 
10, 2011. If you have no input at this time a "no comment" response 
would be appreciated but is not required.
