[G12] New Checklist for G12 analyses

Greetings,

After receiving feedback from my first attempt of creating the checklist 
for g12 analyses, I have uploaded version 2.

Please let me know which one is more acceptable and I will remove one.

Also, in version 2, I have added a editable "Author Remarks" field. I 
would like some feedback on this, on whether you think its a needed.
