Statement of Lessons Learned

Pre-use inspections prove valuable when they uncover an unacceptable condition.  Performing them properly and following up on needed corrections before continuing work is important. Discussion of Activities 
Prior to manipulating a heavy door onto a piece of equipment, the employee inspected the rigging and noticed that the shackle and lifting eye were worn to the point of potential failure. Analysis 
Conditions such as these can develop and go unnoticed whenever rigging is left connected to equipment.  The employee�s pre-use inspection revealed a potential rigging failure that could have resulted in injury and/or equipment damage. Recommended Actions 
Below are the consequences of the above event.  They are provided for information and discussion purposes only. 


Verifying that all components involved in an assignment, including: the task-hazards analyses, equipment, tools, facilities, personnel, training, qualifications, lessons learned from previous iterations, and work instructions are correct for the task and in good working order before starting.
 
For hoisting and rigging:
�	Utilize all lift points in accordance with manufacturer instructions. 
�	Inspect all equipment including rigging hardware, prior to use.  Tag out any item with damage until it is repaired or otherwise suitable for use.
�	Use in accordance with manufacturer instructions
