[Halld-cal] [New Logentry] Bad fadc in FCAL12

There is one more bad fadc module found in the rocfcal 12 crate (D2-7-BOT), slot 5 with the same symptom; failure to connect to the DAC chip.

The bad board (ACDI-476) was replaced by ACDI-058.

Firmware installed:
Ctrl:    241
Proc:  C0D

We took noise/cosmic data over night with the FCAL trigger (read out FCAL only).
Run 40231, 2.4 M events
Trigger rate 50 Hz (I had problems setting all voltages on FCAL)
