[Halld-level1] TS crate for CDC/FDC

Thanks Chris,

I used roc names in the table as it directly shows how
readout crates are connected, this is very convenient
for DAQ. I agree, it would make sense to update the
port map as well.

We are not changing any trunk connections to the patch pannel.
Please swap bcal N6 with S6 connections for the SSP in slot 8
in your table (that's how they are connected, we didn't change
them).
