If your name appears below, you have still not filled out the Foreign Government Sponsored or Affiliated Activities form. The deadline was extended due to previous confusion and the new deadline is Feb 28. Here are the notes from Curtis with relevant links.



Dear Colleagues,

 yesterday Elton sent a reminder that the people listed below have not completed their the disclosure form for Foreign Government Sponsored or Affiliated Activities. If it is not done by Sunday, your access to JLab (including computers) may be turned off. The training link is in the following announcement

Dear Colleague:

On December 15, 2020, the Chief Operations Officer of the Lab sent a message to all staff requesting disclosure of any Foreign Government Sponsored or Affiliated Activities.

You are receiving this email because the following list of employees and/or users who report to you have not completed the disclosure, or confirmed their continued affiliation with an institution or company working under an acceptable agreement to certify their exemption from the disclosure requirement. Failure to finalize the disclosure form or certify the exemption by Sunday, February 28, 2021, will result in the suspension of physical site and/or remote network access. In the event that a computer account is disabled, your users/direct reports will need to work with you and the JLab Help Desk to have their account re-enabled. Once the account is re-enabled, they will have until the close of business to submit the FGSSA form or else their access will be revoked again.
