[Hybrid baryons] PAC44 submission form

Folks,

Attached is my first attempt to fill out the required PAC proposal submission form. I
filled this out for the KY proposal, but most of the form will be identical for the hybrid
proposal. I still need to finalize the disk storage requirements and the computing needs
and will do that early next week.

In the mean time, let me know if you have any feedback on the attached form.

Note that each proposal that is part of a new run group must be submitted separately.
I will complete the submission for the KY proposal, Annalisa will complete the
submission for the hybrid proposal, and Francois will complete the submission for the
DVCS proposal. Once we converge on the information to be filled in on the form I will
send it to Annalisa and Francois so that we all have the same information.

On Thursday I will provide Annalisa with the final version of the hybrid proposal for
submission, along with the author list in the proper format and a copy of the proposal
submission form filled out so that she can enter it into the web interface. Let me know
if you have any comments or questions.
