Attached is a form provided by Walmart for in store tax exemption set up. I would also take our Tax Exempt Certificate. Both are attached. 
For the Walmart provided form, I filled in as much as I can, then you will fill in your information in the store. Once you are in their data base and have the tax exempt card, you should be good to go.

Turns out if you want to purchase on line, you need to set up your own tax exempt account. See below the link to do this and a Tax Exempt certificate is attached. You should get an email approval within 24 to 48 hours.

Good morning all 
I did set us up for Tax exemption but this may be only for on line purchases it seems. See below the steps to take when making a PCard purchase in the store. I have attached a completed tax exempt certificate that you can take with you to be issued a tax exempt card for use in Walmart. You should be able to use your laminated card but in case you need it, certificate is attached.

T o purchase tax exempt items in our Walmart Stores : Our brick and mortar stores have a separate tax exemption system from Walmart.com. If you would like to obtain tax exempt status for our store locations, please take a copy of your applicable tax exempt documents to your local Walmart to be issued a unique Walmart Customer ID number and card. Once you have that card, you will provide it to your cashier when purchasing your items at the store. 

Tax charged on Marketplace items: Items listed on Walmart.com may be sold directly by Walmart or by Marketplace Sellers. 

How to know if it is a Marketplace item – under the price of the item on the website listing or on the order confirmation, there is a line that states “Sold & shipped by {Seller name}”, any company other than Walmart will be a Marketplace Seller. 

If you were charged tax by a Marketplace Seller, be advised that your tax exempt status with Walmart.com currently does not apply to their items and you will need to contact the seller directly to request a refund of the sales tax. You can find the Marketplace Seller’s contact information by clicking their name which will be in blue font on the upper right hand side of the product and/or order page (see screen shots below); once you click on their name, the contact information including the email address and phone number will be displayed on the right hand side of the page as seen in the screenshot. 

Once you click on the “Marketplace Name” highlighted in blue, you will see the below screen. You can then see their phone number and you can email them directly by clicking on email this seller.
