Subject: Spark from pressure gauge caused University of Hawaii explosion

All, 

Attached is an article from the Chemical & Engineering News about a 
University of Hawaii post-doc who lost her arm in an explosion in her lab. 
She was using a pressure gauge not rated for work with flammable liquids. 
Apparently she pointed out to her supervisor that she had experienced static 
shocks and a "small internal explosion" on a smaller vessel and was told not 
to worry about it. The photos are impactful. 

At the DSC meeting we discussed an event here where the employees were 
hesitant to bring up safety concerns because they felt that management 
had not responded to concerns raised previously. Investigation into this 
comment revealed that management was addressing these concerns, but 
corrective actions were going to take some time. The managers had not 
communicated this status back to the employees. I will be sending an 
e-mail to supervisors regarding the lesson learned from our event. 
Perhaps you can use this more significant event to help drive home this 
important message about responding to worker concerns.
