Good afternoon,

The SRF shipping form in Pansophy has been causing errors recently and Valerie has tasked me with a rework of the form to prevent these issues.

Here is the link to the existing form for reference https://pansophy.jlab.org/pansophy/Facilities/SRFOPS_ShippingForm.cfm.

The completed rework has been split into two pages, a request page allowing users to view existing reports they have submitted and an admin page allowing your team to view and close all submitted requests. All existing fields were used to create this new version.

I would like to request your team to review the updated form and provide any feedback (usability issues, errors, etc.) prior to removing the old form and adopting the new one. The links are below.
