[Swreq] ATLis: 1C12 OTR / SLM - device installation and cable pull

For the 1C12 SLM / OTR device, we will need controls to insert the SLM, 
OTR, and Optics using an air solenoid actuator.  This is similar to how 
a standard viewer inserts.  We will also need to know the positioning of 
the devices using limit switch read backs. Please read the ATLis for 
more details.  Below are the requested channel assignments for 1C12.

The video infrastructure is still in the development stage.  We plan on 
using a network camera and implementing it into our SLM server. More
